A spokesperson for the Judicial Conduct Investigations Office said:
The Lord Chief Justice, with the Lord Chancellor’s agreement, have issued Her Majesty’s Senior Coroner Tony Williams with a reprimand following a severe delay in holding an inquest and a failure to keep interested parties informed. In reaching their decision, they took into account the administrative burden created by covid, the coroner’s ill health and his apology to the parties.
